Ham Lane, Lodi <br />APN 059-250-03, SR0085552 <br />PROGRAM COORDINATORS <br />Robert McClellon, REHS <br />Jeff Carruesco, REHS, RDI <br />Willy Ng, REHS <br />Steven Shih, REHS <br />Michelle Henry, REHS <br />Elena Manzo, REHS <br />The San Joaquin County Environmental Health Department (EHD) has reviewed the proposed Onsite <br />Wastewater Treatment System (OWTS) leachfield addition for the above noted location and has the <br />following comments for your consideration. <br />Based on the information provided, the EHD concurs with the proposed engineer design septic system, <br />but requires the following conditions: <br />Submit to EHD an Operation and Maintenance Plan (O&M) for the operator of the OWTS before <br />a septic permit can be issued. The O&M should at the minimum include operating instructions, <br />and recording perimeters for regular monitoring to ensure the OWTS is functioning as designed, <br />such as Average Daily Flow and Peak Flow, and any required analysis (i.e. Biochemical Oxygen <br />Demands, Dissolved Oxygen, and settling solids at both influent and effluent point) per SJC <br />OWTS Standards 1.12.13, and 7.12. EHD also recommends including a boiler -plate monitoring <br />form with the O&M for record keeping consistency and convenience. <br />2. Obtain Engineer Designed System — Commercial addition/repair application prior to installation of <br />system. The fee is based on the current schedule at the time of payment. The system shall be <br />installed and inspected in accordance with this approved plan. <br />3. In addition to the above -referenced installation permit, an annual operation permit (PE4232) is <br />required per SJC OWTS Standards 2.10.1 and 2.10.2 <br />4. The proposed leachfield area field area shall be barricaded so it cannot be driven over, parked <br />on, or used as a storage area. <br />5.
