(113996, 113998, <br /> 114037, 114343(a)) <br /> #23 Rodents, Insects or Animals Inside Facility MAJOR <br /> OBSERVATIONS:Observed live cockroach on prep table in kitchen and some dead cockroaches on counter at sushi bar. <br /> Observed some live cockroaches on floor in kitchen. Facility is serviced by Official Pest Control monthly and the last service <br /> was 8/3/22. Clean and sanitize areas immediately with at least 100 ppm chlorine and increase pest control measures. <br /> Corrected, the cockroach was removed from the prep table and the prep table and sushi bar counter will be cleaned and <br /> sanitized prior to opening at 3. <br /> CALCODE DESCRIPTION:Each food facility shall be kept free of vermin:rodents(rats,mice), cockroaches,flies.(114259.1, 114259.4, <br /> 114259.5) <br /> #22 Disposal of Sewage/Wastewater <br /> OBSERVATIONS:Currently dirty mop water is disposed of behind facility.
