1.0 INTRODUCTION This Site Health and Safety Plan (HSP), developed in accordance with Federal and California Occupational Safety and Health Administration (OSHA) standards for hazardous waste operations (29 CFR 1910.120 and 8 CCR 5192), establishes Site- specific health and safety protocols for Clear Heart Drilling, Inc. For informational purposes only, this plan may be provided to subcontractors of Clear Heart Drilling involved in activities at the site. Entities and personnel other than Clear Heart Drilling's staff shall be solely responsible for their own health and safety and shall independently assess on-site conditions and develop their own health and safety protocol. However, other entities or personnel that anticipate using health and safety measures which are less stringent than Clear I leart Drilling's measures should immediately conact Clear Heart Drilling Site Safety Officer. Clear Heart Drilling, Inc. has developed a health and safety plan for Clear Heart Drilling's employees. The Corporate Plan complies with current health and safety regulations, including Cal OSHA 8 CCR 5192 and OSHA 29 CFR 1910.120, Hazardous Waste Operations and Emergency Response. Many of the procedures in the corporate plan are conducted on a routine basis (general training, respirator fit testing, general medical record keeping, etc.) and are not repeated in this document. Questions regarding the Clear Heart Drilling Corporate Plan are referred to Terri White, Operations Manager. I A copy of the Site Health and Safety Plan and Standard Operating Procedures along w th specific health and safety information will be kept in a conspicuous location on site at 11 times while work is being conducted. 2.0 KEY HEALTH AND SAFETY PERSONNEL The',Clear Heart Drilling Corporate Safety Officer is Gary Soden. Clear Heart Drilling SitelSafety Officer is listed on page one, titled "Site Specific Health Safety Plan Summary." The Site Safety Officer will be present during field activities at all times. The Site Safety Officer is responsible for the following: • Observing field activities for compliance with this Site Health and Safety Plan, as it applies to the extent of work on site that specifically applies to Clear Heart Drilling • Modifying health and safety protocols or terminating fieldwork when unsafe work conditions exist • Familiarizing Clear Heart Drilling's personnel with health and safety protocols • Ensuring that Clear Heart Drilling's field personnel wear appropriate personal protective equipment j Page 1
