The following is an itemized list of hazardous materials violations that have not been <br /> addressed for GUZMAN'S AUTO REPAIR as of September 15, 2022. <br /> Open violations from July 22, 2021 inspection <br /> Violation#5-Failed to electronically submit hazardous materials inventory for all reportable materials on <br /> site. <br /> OBSERVATION: Used oil 275 gallons of used oil were found on site that have not been reported to CERS.The <br /> max daily amount for new oil should be 55 gallons not 5 gallons. The max daily amount for used anti freeze should <br /> be 55 gallons not 5 gallons. <br /> REGULATION GUIDANCE:Any material that meets or exceeds the reportable quantity shall be reported in the <br /> facility's business plan. <br /> CORRECTIVE ACTIONS: Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/, add the materials to the hazardous material inventory, and submit to the EHD for <br /> approval. <br /> ❑ This violation was corrected ❑This violation will be corrected by(date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation#6-Failed to electronically submit a site map with all required content. <br /> OBSERVATION: The site map is incomplete. The site map needs acronyms for SD and FE defined in a key or <br /> legend on the site map. <br /> REGULATION GUIDANCE:A site map shall contain a north orientation, loading areas, internal roads, adjacent <br /> streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous <br /> material handling and storage areas, and emergency response equipment. <br /> If a site map element is not applicable for your facility then list it on the map and label as"NA". <br /> CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br /> ❑ This violation was corrected ❑This violation will be corrected by(date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Page 1 of 1 <br />
