CONTRACTOR NOTES (FOR ALL SHEETS): <br />1. ALL TRAFFIC CONTROL DEVICES SHALL CONFORM TO THE LATEST EDITION OF CALIFORNIA MANUAL <br />OF UNIFORM TRAFFIC CONTROL DEVICES (CA MUTCD), THE CALIFORNIA TEMPORARY TRAFFIC <br />CONTROL HANDBOOK (CATTCH), AND CALTRANS STANDARD PLANS AND SPECIFICATIONS. <br />2. THE LOCAL FIRE DEPARTMENT, SHERIFF/POLICE DEPARTMENT, CALIFORNIA HIGHWAY PATROL, <br />LOCAL SCHOOL DISTRICT, TRANSIT SERVICE PROVIDERS AND UNITED STATES POSTAL SERVICE <br />SHALL BE NOTIFIED 72 HOURS IN ADVANCE OF CONSTRUCTION WORK. <br />T THE CONTRACTOR IS SOLELY RESPONSIBLE FOR THE REPAIR OF ANY DAMAGE TO EXISTING <br />IMPROVEMENTS RESULTING FROM THE CONTRACTOR'S OPERATION. THIS REPAIR SHALL INCLUDE <br />ALL CONSTRUCTION AND MATERIALS TO RESTORE THE SITE TO EXISTING CONDITIONS. <br />4. ALL ADVANCED WARNING SIGNS SHALL BE EQUIPPED WITH A HIGH-LEVEL WARNING DEVICE (FLAG <br />TREE). <br />5. ALL WORKERS SHALL BE EQUIPPED WITH A REFLECTIVE VEST AND HARD HAT PER LATEST ANSI/ISEA <br />107 STANDARDS. ALL FLAGGERS SHALL ALSO BE EQUIPPED WITH A R1 -1/W20-8 "STOP/SLOW" PADDLE <br />AND SHALL BE TRAINED IN THE PROPER FUNDAMENTALS OF FLAGGING TRAFFIC. <br />6. NO PARKING SIGNS SHALL BE PLACED 72 HOURS PRIOR TO SET UP. SIGNS SHALL BE POSTED EVERY <br />20 LINEAR FEET OF OCCUPIED SPACE WITH AT LEAST ONE SIGN AT EACH END OF OCCUPIED SPACE. <br />7. ANY CONFLICTING SIGNS WILL BE COVERED FOR THE LENGTH OF THE JOB. <br />8. CONTRACTOR SHALL MAINTAIN EMERGENCY VEHICLE AND DRIVEWAY ACCESS AT ALL TIMES. <br />9. PACIFIC GAS & ELECTRIC COMPANY ASSUMES NO LIABILITY WHEN CONTRACTOR DOES OWN SET UP <br />OR MAKES CHANGES TO THE ORIGINAL TRAFFIC CONTROL PLAN. <br />10. CITY ENGINEER OR HIS REPRESENTATIVE HAS THE AUTHORITY TO INITIATE FIELD CHANGES TO <br />ASSURE PUBLIC SAFETY. <br />11. CONTRACTOR SHALL PLATE EXCAVATED AREA DURING NON -WORKING HOURS. <br />12. CONES SHOWN ON PLAN ARE FOR ILLUSTRATION PURPOSES ONLY. EXACT NUMBER OF CONES <br />REQUIRED SHALL BE BASED ON THE POSTED SPEED, TANGENT LENGTHS, TAPER LENGTHS, <br />CONFLICTING SECTIONS, AND ACTUAL FIELD CONDITIONS. <br />13. PLAN IS ACCURATE FOR TRAFFIC CONTROL WORK ONLY. EXCAVATION LOCATION IS APPROXIMATE <br />AND FOR REFERENCE ONLY. <br />14. LIGHT TOWERS SHALL BE USED AT FLAGGER STATIONS AND WORK AREAS WHEN WORK IS AT NIGHT <br />OR ENCROACHES THE HOURS OF DARKNESS. HIGHWAY CONSTRUCTION WORK LIGHTING SHALL BE <br />PER CALIFORNIA CODE OF REGULATIONS CONSTRUCTION SAFETY ORDER 1523 (TILE 8, DIVISION 1, <br />CHAPTER 4, SUBCHAPTER 4, ARTICLE 3, SECTION 1523). <br />15. NOTHING ALLOWED IN BUFFER OR TRANSITION AREAS UNLESS OTHERWISE SHOWN ON THE PLAN. <br />16. TYPE A OR TYPE B FLASHING WARNING LIGHTS SHALL BE USED, AS SHOWN ON THE PLAN, WHEN <br />WORK IS AT NIGHT OR ENCROACHES THE HOURS OF DARKNESS. TYPE B FLASHING WARNING LIGHTS <br />SHALL BE USED, AS SHOWN ON THE PLAN, WHEN WORK IS 2417 SETUP. <br />17. IF PEDESTRIAN ACCESS IS RESTRICTED DURING WORKING HOURS, AT LEAST ONE WORKER SHALL BE <br />ASSIGNED WITH THE RESPONSIBILITY TO SAFELY ESCORT PEDESTRIANS IN DEED OF ASSISTANCE. <br />THE ASSIGNED WORKER(S) MAY ALSO PARTICIPATE IN OTHER CONSTRUCTION ACTIVITIES BUT SHALL <br />ALWAYS BE AWARE OF HIS/HER RESPONSIBILITY TO PROVIDE THIS ASSISTANCE. PEDESTRIANS <br />SHALL NOT BE DIRECTED NOR EXPECTED TO CROSS A ROADWAY AT ANY LOCATION OTHER THAN AT <br />A SIGNALIZED INTERSECTION OR ALL -WAY STOP. ACCOMMODATIONS SHOULD BE MADE ON THE <br />SAME SIDE OF THE ROADWAY AS THE WORK. <br />18. WHEN APPLICABLE, PLACE A MINIMUM OF FOUR CONES AT 50FT INTERVALS IN ADVANCE OF FLAGGER <br />STATION. <br />19. WHEN APPLICABLE, PLACE SHOULDER TAPER IN ADVANCE OF PCMS BOARD AND/OR PORTABLE <br />RADAR SPEED FEEDBACK SIGN SYSTEM. SEE CALTRANS STD T9 TABLE 1 FOR SHOULDER TAPER <br />LENGTH. <br />20.
