The test <br /> results on site are dated 9/2/2020 <br /> REGULATION GUIDANCE: These records shall be maintained on site for at least three years. Monitoring records <br /> include: (1)date and time of all monitoring or sampling; (2)monitoring equipment calibration and maintenance <br /> records; (3)results of any visual observations; (4)results of sample analysis performed a lab or in the field; (5)logs <br /> of all readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6)results of <br /> inventory readings and reconciliations. <br /> CORRECTIVE ACTION: Immediately locate and maintain all missing maintenance and monitoring records for the <br /> last three years on site. Submit copies of any missing work orders and alarm records to the EHD. <br /> This is a minor violation. <br /> 306 HSC 25291(e)Failure to remove, analyze, and properly dispose of water in secondary containment(pre-Jul 2003). <br /> OBSERVATION: Liquid was observed in the 87 and 91 spill and vapor containers. <br /> REGULATION GUIDANCE: If water could enter into the secondary containment by precipitation or infiltration, it <br /> must be removed and disposed of properly. <br /> CORRECTIVE ACTION: The service technician removed approximately 5 gallons of liquid from the 87 and 91 spill <br /> and vapor containers. Ensure that all sumps and annular spaces are maintained free of liquid. <br /> This was corrected on site. <br /> This is a minor violation. <br /> 317 CCR 2636(f)(2) Failure of the line leak detector to detect a 3.0 gph leak at 10 psi and restrict or shut off flow. <br /> OBSERVATION: The 87 VMI line leak detector failed to detect a leak when tested. <br /> REGULATION GUIDANCE:All line leak detectors shall be capable of detecting a 3-gallon per hour leak at 10 psi. <br /> CORRECTIVE ACTION: The service technician adjusted the leak detector, retested it, and it passed. <br /> This was corrected on site. <br /> This is a Class II violation. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by 01/14/2023 . <br /> To minimize person to person contact EHD is choosing to write the name of person receiving the report instead <br /> FA0000916 PR0231435 SCO01 12/15/2022 <br /> EHD 23-01 Rev.12/06/2021 Page 5 of 6 Underground Storage Tank Program OIR <br /> 1868 E.
