When she bit into it she chipped her tooth with a bread tag that was in her sandwich. <br />VIOLATIONS AND CORRECTIVE ACTIONS <br />Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7; <br />113700. All violations must be corrected within specified timeframe. Violations that are classified as "MAJOR" pose an immediate threat to public health <br />and have the potential to cause foodbome illness. All major violations must be corrected immediately. Non-compliance may warrant immediate closure of <br />the food facility. <br />OVERALL INSPECTION NOTES AND COMMENTS <br />FOOD ITEM -- LOCATION --TEMP ° F -- COMMENTS <br />No Temperature Data Collected <br />COMMENTS AND OBSERVATIONS <br />On site to conduct complaint investigation. Per Myrna Barut (Shift Manager), facility is aware of the complaint and District <br />Manager and General Manager are working on it. Buns for chicken sandwiches come in plastic bags with bread tags. Bread <br />tags are removed and disposed of prior to being brought to sandwich line. Did not observe any foreign objects in sandwich <br />prep line ingredients. Observed two sandwiches being made. Did not observe any loose debris on the ceiling or surrounding <br />area of the sandwich prep line. Observed four blue bread tags on floor by the mop sink. <br />Unable to verify alleged complaint. <br />The person in charge is responsible for ensuring that the above mentioned facility is in compliance with all applicable sections of the California Health and <br />Safety Code.
