<br /> OBSERVATIONS:Observed cardboard on shelves in 5 dr display and market walk-in cooler. Remove cardboard and

<br /> Observed debris on soda syrup hoses. Clean today and maintain clean.
<br /> Observed debris on exhaust hood filters. Clean within 1 week and maintain clean.
<br /> Some of the lights in the display warmer were not working. Provide working lights within 2 weeks.
<br /> The deflector plate in the ice machine was damaged. Replace deflector plate within 3 weeks.
<br /> The entrance door for the market walk-in was damaged, observed taped around door. Replace door or repair with cleanable
<br /> material within 3 weeks.

<br /> OBSERVATIONS:The toilet in restroom was not flushing. Per employee they will have someone come out today to fix the
<br /> toilet, currently a portable toilet located on the side of the facility is used. Repair and provide working toilet immediately.

<br /> well-fitting self-closing door. Toilet tissue shall be provided in a permanently installed dispenser at each toilet. The number of toilet
<br /> facilities shall be in accordance with local building and plumbing ordinances. Toilet facilities shall be provided for patrons:in
