SAN JOAQUIN <br /> Environmental Health Department <br />( I() 1..AN i <br />Time In: 12:00 pm <br />Time Out: 1:11 pm <br />COMPLAINT INSPECTION REPORT <br />Name of Facility: EL JARDIN RESTAURANT Date: 09/15/2022 <br />Address: 1119 S MAIN ST, MANTECA 95337 <br />Owner/Operator: BLUETT, CHRISTOPHER Telephone: (209) 825-8552 <br />Program Element: 1600- FOOD PROGRAM Complaint #: C00055230 <br />Responsible Party Contact: <br />DESCRIPTION OF COMPLAINT ALLEGATIONS <br />The San Joaquin County Environmental Health Department received the following complaint: <br />COMPLAINANT IS A 62 YEAR OLD MALE THAT WENT TO FACILITY ON 9/6/2022 AROUND 6:00PM. COMPLAINANT <br />WAS ONLY ONE IN PARTY TAHT BECAME ILL AND THE ONLY ONE THAT ATE CHICKEN QUESADILLA. ATE FOOD <br />AT RESTAURANT BECAME ILL TWO HOURS LATER WITH V OMITTING AND DIARRHEA. WENT TO ER NEXT NIGHT <br />AND DIAGNOSED WITH CAMPYLOBACTER. COMPLAINANT WAS STILL ILL THREE DAYS LATER AND RETURNED <br />TO ER AND WAS ADMITTED TO HOSPITAL FOR THREE DAYS. COMPLAINANT IS FEELING BETTER AT TIME OF <br />COMPLAINT. <br />VIOLATIONS AND CORRECTIVE ACTIONS <br />Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7; <br />113700. All violations must be corrected within specified timeframe. Violations that are classified as "MAJOR" pose an immediate threat to public health <br />and have the potential to cause foodbome illness. All major violations must be corrected immediately. Non-compliance may warrant immediate closure of <br />the food facility. <br />#1 Demonstration of Knowledge <br />OBSERVATIONS: 8 food handler cards are not available on site. Provide valid food handler cards to gfahmy@sjgov.org in <br />1 week. <br />CALCODE DESCRIPTION: All food employees shall have adequate knowledge of and be trained in food safety as it relates to their <br />assigned duties. (113947) Food facilities that prepare, handle or serve non-prepackaged potentially hazardous food, shall have an <br />employee who has passed an approved food safety certification examination. (113947-113947.1) Any food handler hired after June 1, <br />2011 shall obtain a Food Handler Card within 30 days (113948). <br />#7 Hot and Cold Holding Temperatures <br />OBSERVATIONS: Walk in cooler has temp at 47F. Provide 41F or below today. <br />CALCODE DESCRIPTION: Potentially hazardous foods shall be held at or below 41/45°F or at or above 135°F. (113996, 113998, <br />114037, 114343(a)) <br />#34 Warewashing Facilites Maintained <br />OBSERVATIONS: Chlorine test strips are not available. Provide strips in 1 week. <br />CALCODE DESCRIPTION: Food facilities that prepare food shall be equipped with ware washing facilities. Testing equipment and <br />materials shall be provided to measure the applicable sanitization method.
