I asked Vicky to gather more information about <br />the Ansul system and power. Also, to tell the owner not to open until he is <br />inspected by EHD. <br />02/08/2022 I received a page from the Lodi Fire Department at 4:24am. I contacted operator <br />#14 (Kristen) of the fire department who reported that there was a fire at 2312 W. <br />Kettleman Ln in Lodi, CA, Per Kristen, foam was used to extinguish the fire. <br />I contacted the facility's primary emergency contact listed in CERS, Sherry <br />Witherow (area manager), who reported that she was aware of the fire and stated <br />that the store general manager Ivan Vaga was on site now. I contacted Ivan at <br />909-504-8069. Ivan stated that he is on site currently with the fire department. <br />Ivan states that there is damage to the kitchen and office areas and he will be on <br />site to board up the doors once the fire department is done. Ivan states all power <br />has been shut-off to the building and he was unsure if the ansul system was <br />triggered during the fire. <br />I received a call from Battalion Chief Shane Langone reporting that approximately <br />1-2 gallons of Firebull Foam was used with 300-500 gallons of water to extinguish <br />the fire. Per Shane, the foam used is environmentally friendly. <br />I called to update lead Haza Saeed. <br />VAN G-LEE <br />(4:24am - Sam) <br />02/08/2022 Updated leads and program coordinator (Haza Saeed, Elena Manzo, and Melissa VANG-LEE <br />Nissim). I also notified the food inspector Stephanie Ramirez of the fire. <br />02/08/2022 Generated a complaint and reviewed SDS for Firebull foam. VANG-LEE <br />03/29/2022 Updated complaint record with the responsible party's information. VANG-LEE <br />07/19/2022 Complaint was on the Unresolved Complaint list. RAMI REZ <br />Complaint abated.
