Complaint Investigation Summary <br /> Report #. 5105 <br />COMPLAINT ID: COUMCKXZ1 Site Location: 6011 E PINE ST <br />Activity Summary <br />Activity Date Recorded by <br />05/16/2019 At 8:15am Garrett Backus informed me that he received a complaint from an SAEED <br />employee, who works at All State Packers in Lodi. The complaint stated, "east side <br />packing line (there are three total) has an ammonia smell. Operator is distributing <br />masks but is taking no other corrective actions." <br />At 9:10am today, I arrived at the facility and met with the CEO, Patrick Archibeque. <br />We discussed the reason of the visit. I was told by Patrick there was no evacuation <br />or exposure. <br />I inspected the area where the release was coming from. <br />Ammonia vapor released out of the aqueous ammonia system for the hydrocooler <br />located outside on the east side of the facility due to a defective shaft seal for the <br />pump. arrived onsite and the ammonia system was still running, and ammonia odor <br />is still present. Jeremy Mostin with ICR/refrigeration contractor was onsite to <br />correct the issue. ICR has ordered a shaft seal, and will arrive around 1pm. i asked <br />the facility to shut off the system, and resume after the issue is corrected. Jeremy <br />told me this an aqua ammonia not anhydrous ammonia and it is not hazardous. I <br />explained it is extremely hazardous substance, and the system need to be shut off, <br />and resume after correcting the issue. ICR shut off the system, and the shaft seal <br />will be replaced today. the facility is considering replacing this part of the system <br />with a glycol system. <br />At 12:15pm I stopped by the facility again to check if the shaft seal was replaced. I <br />met with the Chief operator officer, Peter V. Craig and was told that ICR is out for <br />lunch, but they will come back and replace the shaft seal by 2pm today. I was told <br />by Peter that this Denk System was installed last year, and they have had <br />problems with the shaft seal for the pump, and therefore they will replace this <br />system by a glycol system. I discussed with Peter that they need to update their <br />RMP and submit a copy of the update to me. I told peter I may come back today or <br />tomorrow to inspect the system after the shaft seal replacement. I asked Peter if <br />there were any complaints from the employees or any exposure or if anyone <br />required medical attention. Peter told me that one employee complained of having <br />watery eyes, but returned to work without any medical attention. I left the site at <br />12:50pm. <br />A notice to abate was emailed to Patrick and Cc'd Peter at 2:57pm. The notice to <br />abate included the following corrective actions: <br />Report all the accidental ammonia releases to SJC EHD and CAL OES <br />immediately <br />Submit all the risk management plan (RMP) documents for adding/installing the <br />Denk system/ammonia system for the hydrocooler within 3o days. <br />I completed the narratives at 3:17pm and updated Garrett Backus with the incident <br />status. <br />Discussed the incident update with Muni at 3:20pm. Muni Advised to inform Dr. <br />Baldwin of the employee complained of a watery eyes. <br />I emailed Dr. Baldwin with SJC PHS stating, <br />"The San Joaquin County Environmental Health Department received a complaint at <br />8:15 am today for an ammonia release (unknown amount) to the air at All State <br />Packers, 6011 East Pine Street, Lodi. When I inspected the facility this morning, I <br />was told there was no evacuation or exposure. However, When I returned to <br />follow-up on the complaint around noon today I was told that one employee <br />complained of watery eyes, but return to work without seeking any medical <br />attention.
