QUALITY ASSURANCE AND QUALITY CONTROL PLAN <br />The Quality Assurance/Quality Control Plan (QAQCP) is intended to facilitate the acquisition of accurate and <br />reliable data. Environmental data gathered during the groundwater sample collection work will be collected <br />and analyzed following procedures prescribed in the Quality Control Plan. A Quality Assurance Plan has <br />been established to assure that the Quality Control Plan is effective. As required all samples collected during <br />the scope of work will be submitted to California Department of Health Services (DHS) Environmental <br />Laboratory Accreditation Program (ELAP) certified laboratories for analysis following ASTM D 4840 chain <br />of custody procedures. The following field and laboratory procedures will be implemented to ensure that <br />QAQCP objectives are met. <br />1.0 FIELD DOCUMENTATION AND INVESTIGATION REPORTING <br />All information pertinent to the scope of work will be kept in a field log book/daily log. All field <br />documentation forms will be recorded in ink and maintained for five years following the conclusion of the <br />project. No documentation will be destroyed or discarded. If mistakes or errors are made on any field <br />documentation form, the error will be crossed out using a single line through the incorrect data leaving the <br />mistaken entry legible and the correction will be initialed and dated. All analytical data will be reviewed and <br />validated for quality control by the accredited laboratory and Ground Zero Analysis, Inc. (Ground Zero). A <br />report of findings and analytical testing will be prepared by Ground Zero following the completion of the <br />groundwater sample collection. Information to be documented and reported includes at least the following: <br />- Sample numbers <br />- Locations of sample collection <br />- Soil boring or well numbers, as applicable <br />- Depths at which samples were obtained <br />- Names of collectors <br />- Dates and times of collection <br />- Purpose of sample <br />- Sample distribution (e.g. laboratory, archive, etc.) <br />- Field observations <br />- Field measurements (e.g. PID readings, pH, conductivity, water levels.) <br />- Other data records (e.g. development log, soil sampling report, well log, etc.) <br />2.0 LABORATORY ANALYTICAL METHODS <br />The primary analytical laboratory will be: <br />1. Pace Analytical Environmental Services of Bakersfield, California (ELAP #1186) for the analysis of <br />the groundwater, soil and soil vapor samples <br />Groundwater samples will be collected in 40mL glass VOAs preserved with HCl provided by Pace <br />Analytical.
