TRAFFIC HAZARDS <br /> Field activities taking place in public streets or on road shoulders shall use Caltrans approved flagging and <br /> traffic control during the duration of activities. While working in public streets with heavy traffic assume <br /> that the operator of a motor vehicle is unable to see you and keep a safe distance from moving traffic. Keep <br /> off roadways, when practical, and away from moving traffic. Approved safety vests will be worn by all <br /> personnel when working in areas of traffic hazard. <br /> COMMUNICATION PROCEDURES <br /> The following standard hand signals will be used if and when required. <br /> Hand gripping throat................Out of air,can't breathe <br /> Grip partner's wrist or <br /> both hands around waist...........Leave area immediately <br /> Hands on top of head................Need assistance <br /> Thumbs up................................OK,I'm all right,I understand <br /> Thumbs down...........................No,negative <br /> EMERGENCY RESPONSE PLAN <br /> On-site personnel will use the following standard emergency procedures. The Site Safety Officer shall be <br /> notified of any on-site emergencies and be responsible for ensuring that the appropriate procedures are <br /> followed. Emergency Procedures should be modified as required for the incident. <br /> Personal injures Upon notification of an injury,the Site Safety Officer will assess the nature of the <br /> injury. If the injury is in the area of the drilling or excavating equipment, the equipment shall be <br /> shut down to eliminate running equipment hazards.Equipment will remain shut down until the Site <br /> Safety Officer authorizes work to proceed. If the cause of the injury or loss of the injured person <br /> does not affect the performance of site personnel, operations may continue. <br /> Fire/Explosion: In the event of a fire or explosion on site,the Site Safety Officer or alternate shall <br /> call 911 (the fire department)and all personnel shall be moved to a safe distance from the involved <br /> area. <br /> Personal Protective Equipment Failure: If any site worker experiences a failure or alteration of <br /> protective equipment that affects worker's protection,that person shall immediately proceed to the <br /> nearest staging area to repair or replace the damaged equipment. The person shall not return to the <br /> work area until the protective equipment is repaired or replaced.If the equipment cannot be repaired <br /> or replaced,the worker will proceed to the primary staging area specified by the Site Safety Officer <br /> or leave the site. <br /> Other Equipment Failure: If any other equipment on site fails to operate properly, the Site Safety <br /> Officer shall be notified and then determine the effect of this failure on continuing operations on <br /> site. If the failure affects the safety of personnel or prevents completion of the work plan tasks, all <br /> personnel shall leave the work area until the situation is evaluated and appropriate actions taken. <br />
