<br />Page 5 of 10 <br />Drill rig cables, hoses, and ropes are to be safely secured and periodically checked for abrasion or defect. <br />Unsafe equipment shall be replaced immediately. Other equipment will be safely stored and secured when <br />not in use. <br /> <br />The Site Safety Officer shall review with all personnel and subcontractors the equipment inspection checklist <br />for company vehicles. The Site Safety Officer shall also confirm that the drill rig operator has performed a <br />pre-startup safety inspection/observation prior to the start of work and that adequate water is available at the <br />rig for emergency decontamination and washing in case a worker is exposed to contaminated water or soil. <br /> <br />Field vehicles shall park in an area that is a safe distance from operating equipment and vehicle traffic with <br />the location approved by the Site Safety Officer. Care must be given when approaching heavy equipment. <br />Personnel on foot should approach from a direction facing the operator and not a blind side. Eye-to-eye <br />contact should be made before proceeding within the working limits of the operator and their heavy <br />equipment. <br /> <br />OVERHEAD HAZARDS <br />Overhead hazards include utility lines and trees. Prior to performing work requiring the operation of <br />equipment above eye level, the evaluation of overhead hazards shall be made. Equipment shall be located <br />to maximize the clearance from overhead hazards (minimum 10 feet clearance). <br /> <br />UNDERGROUND UTILITIES <br />Condor personnel have contacted Underground Service Alert prior to drilling or excavation activities. The <br />property owner is responsible for marking the location of their own underground utilities. The approximate <br />location of underground utilities shall be marked on the existing ground surface. A minimum of 3 feet <br />clearance shall be given to marked utilities. If drilling or excavation near utilities is required, hand <br />excavation and probing shall be performed in 6-inch intervals prior to drilling or excavating activities. <br />Ground Penetrating Radar (GPR), will also be utilize the locate any existing USTs or other underground <br />utilities in the area. <br /> <br />MOVING AND ROTATING MACHINERY <br />Drilling activities involve the use of large, heavy, moving, and rotating equipment. Loose fitting clothing <br />that can be caught or snagged should be avoided. Work gloves should be worn while working with moving <br />metal parts and equipment. <br /> <br />PHYSICAL HAZARDS <br />CONFINED SPACES/EXCAVATIONS <br />Condor employees are not authorized to enter confined spaces without prior written approval from Condor’s <br />Corporate Safety Manager or designee. Excavations can be considered confined spaces. Employees are not <br />authorized to enter excavations deeper than 5 feet without proper authorization and without proper shoring <br />of excavation sides approved by a geotechnical engineer. Samples of excavations should be taken from <br />material in the excavator bucket after it is brought to ground surface. Proper barricades shall be placed <br />around open excavations when work is not in progress. <br /> <br />FALL PROTECTION <br />The Site Safety Officer will verify that all Condor employees and its subcontractors are using appropriate <br />fall protection. The three major components of personal fall arrest protection are harness, lanyard, and <br />secure anchorage. OSHA requires that fall protection be used anytime someone is working over six feet <br />above the ground or the floor. MSHA requires that fall protection be used anytime a risk of falling exists. <br /> <br />TRIP AND SLIP PROTECTION <br />Good housekeeping shall be maintained at the site to prevent trip and slip hazards.
