LANDFILL ENVIRONMENTAL HEALTH <br /> PERMIT/SERVICES <br /> Thank you for inquiring about disposing of contaminated soils <br /> at the Forward, Inc. Landfill. We have enclosed the following <br /> materials: <br /> A Waste Characterization Form <br /> • A Contaminated Soil Description Form <br /> • A flow chart showing the decision-making <br /> process for accepting contaminated soils at the <br /> Forward Landfill <br /> In order to process your request, we must receive the <br /> following from you: <br /> • The fully completed Waste Characterization <br /> Form, signed by the generator. <br /> The fully completed Contaminated Soil <br /> Description Form, signed by a California <br /> Registered Geologist or California Professional <br /> Engineer. <br /> ° Copies of all State-Certified laboratory <br /> results for the contaminated soils. <br /> The description of the composition of the contaminated soils <br /> must be based on State-Certified analyses of samples that are <br /> representative of the contamination types and concentrations in the <br /> soils. You must fill out all sections of the Waste <br /> Characterization Form and the Contaminated Soil Description Form. <br /> Indicate "N/A" if a given section of such Forms is inapplicable. <br /> Please send the completed materials to: <br /> Forward, Inc. <br /> P.O. Box 6336 <br /> Stockton, California 95206 <br /> Attn: Environmental Compliance Officer <br /> Please be advised that no hazardous waste will be accepted, <br /> as defined by 40 CFR Part 261 and the California Code of <br /> Regulations, Title 22 (except asbestos) . Contaminated soils that <br /> are not approved for direct disposal may be considered for <br /> treatment in the Landfill 's Class II soil treatment unit. <br /> Forward will promptly evaluate the submitted materials. You <br /> will ,be contacted following that evaluation, or if further <br /> information or clarification is necessary. <br />
