Order of Sample Collection <br />During each sampling event, sample bottles <br />were not filled in the systematic order as <br />specified in the SAP. The sampler appeared to <br />take sample bottles from the cooler according <br />to which one was closest within reach. <br />This deviates from the procedure stipulated in <br />the SAP. (Sample Collection -Sample <br />Withdrawal). Page two of the SAP specifies a <br />prescribed order for individual sample <br />collection. <br />vi. Insufficient Samples <br />TDS, Radionucleides, Coliform Bacteria, POC, <br />and PDX were not included in the sampling <br />parameters. <br />This deviates from the procedure stipulated in <br />the SAP (SAMPLE Collection -Sample Withdrawal). <br />Page two and three of the SAP stipulate that <br />TOS, Radionucleides, Coliform Bacteria, POC, <br />and PDX shall be sampled. <br />Violation 2-: Failure to Maintain Ground Water -Monitoring <br />System <br />Forward violated the post -closure permits issued by USEPA <br />in that on or about October 21, 1991, Forward Inc. failed <br />to maintain the ground water monitoring system at the <br />facility as described in Attachment 3 (Well Design <br />Protocol) of the RCRA Post Closure Permit dated June 3, <br />1991 and Attachment 3 (Well Design Protocol) of the <br />Hazardous Waste Facility Post Closure Permit issued <br />February 2, 1990 by DTSC. Failure to maintain the ground <br />water monitoring system is a violation of State of <br />California Code of Regulations Title 22, Article 6, <br />Section 66264.310 (b) (3) Closure and Post -Closure Care. <br />On October 21, 1990, Will Rowe and Steve Posner of DTSC <br />observed the following examples of inadequate monitoring <br />system maintenance: <br />o MW -3A: Standing water in the steel isolation <br />casing. <br />13 <br />,E 5e �i r <br />
