k% KLEINFELDER <br /> chain-of-custody to an analytical laboratory certified by the state for the planned chemical <br /> analysis. <br /> A-3.3 Sample Handling <br /> After labeling, the sample is immediately stored in an iced cooler for transport to the <br /> analytical laboratory. The label includes Kleinfelder's name, job number, the date and <br /> time the sample was collected, the employee number of the individual who performed the <br /> sampling, and a unique five-digit sample identification number. A custody seal is also <br /> placed on the sample in such a way that any attempt to tamper with the sample is easily <br /> visible. An example of a sample label and custody seal are attached at the end of this <br /> appendix. <br /> A Kleinfelder chain-of-custody form is attached to the cooler. The chain-of-custody form <br /> includes Kleinfelder's name, address and telephone number, the employee number of the <br /> individual who performed the sampling, the sample numbers, the date and time the <br /> samples were collected, the number of containers each sample occupies, and the analyses <br /> for which the samples are being submitted. The chain-of-custody form is signed by each <br /> person who handles the samples, including all Kleinfelder employees and the receiving <br /> employee of the analytical laboratory when the samples are delivered. An example of the <br /> chain-of-custody form is attached. <br /> A-3.4 Equipment Decontamination <br /> To reduce the potential for cross-contamination between wells, all developing and <br /> sampling equipment is washed in a trisodium phosphate solution and rinsed in distillel <br /> Ift—ter or steam cleaned prior to use in the next monitoring well. <br /> A-3.5 Well Survey <br /> The locations of soil borings and monitoring wells and the elevation of the top of the PVC <br /> casings is usually surveyed and tied into permanent markers, if readily available. Survey <br /> accuracy is 0.1 foot for the "x" and "y" coordinates and .01 foot for the "z" coordinate. The <br /> depth to static ground water is measured from a set location at the top of the PVC casing. <br /> The depth of water is then subtracted from the elevation of the top of the well casing to <br /> provide a ground water elevation for each monitoring well location. <br />
