The 2020 submittal was submitted <br /> late ono 4/08/2020. <br /> REGULATION GUIDANCE: The hazardous materials inventory shall be submitted by January 15 of each calendar <br /> year and may be submitted beginning November 1 of the previous year. <br /> CORRECTIVE ACTION: Corrected on-site. Site submitted the 2021 and 2022 submittal on-time. <br /> This is a minor violation. <br /> 11 HSC 25505(a)(4) Failed to provide or document initial and/or refresher training to appropriate personnel. <br /> OBSERVATION: Prove of annual refreshing training documents of the elements listed below were not available <br /> on-site at the time of the inspection. <br /> REGULATION GUIDANCE: The business plan shall include provisions for ensuring that appropriate personnel <br /> receive initial and annual refresher training.All employees shall be trained in safety procedures in the event of a <br /> release or threatened release of a hazardous material, including, but not limited to,familiarity with the following <br /> plans and procedures: <br /> (A) Immediate notification contacts to the appropriate local emergency response personnel and to the unified <br /> program agency. <br /> (B)Procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br /> persons, property, or the environment. <br /> (C) Evacuation plans and procedures, including immediate notice,for the business site. <br /> This training shall be documented electronically or by hard copy and shall be made available for a minimum of <br /> three years. <br /> CORRECTIVE ACTION: Immediately provide employee training for appropriate personnel and submit a copy of the <br /> training records to the EHD. <br /> This is a Class II violation. <br /> 12 HSC 25505(c)Failed to have the HMBP readily available to facility or EHD personnel. <br /> OBSERVATION: The operators at this facility do not have access to emergency response plans/emergency action <br /> plans, chemical inventory and site map. <br /> REGULATION GUIDANCE: Emergency response plans and procedures, chemical inventory, and site map shall <br /> be readily available to personnel of the business or facility with responsibilities for emergency response or training. <br /> CORRECTIVE ACTION: Corrected on-site at the time of the inspection. <br /> This is a minor violation. <br /> FA0019294 PR0528731 SCO01 10/07/2022 <br /> EHD 19-01 Rev.12/06/2021 Page 2 of 3 Hazardous Materials Program OIR <br /> 1868 E.
