In order to protect the ground water <br />supply and your water source from potential degradation, please take the associ 3ted corrective actions. <br />X Lack of or Inadequate Concrete Platform around Wellhead. <br />(SJC Water Weil Standards, Section 4.10. - Surface Seat) <br />Corrective Action: Repair existing or install a new concrete platform around the wellhead. <br />0 Lack of or Inadequate Sanitary Seal at Wellhead. <br />(SJC Ord. Code, Title 9, Section 9-1115,5 - Well Regulations) <br />Corrective Action: Install a sanitary seal between casing and well coter plateiseal all gaps and <br />openings. <br />X Lack of or Inadequate Sampling Tap. <br />(SjC Water well Standards, Section 4 11 - Sampling Tap) <br />Corrective Action: Install a non -threaded sampling tap between the we 11head and the check valve <br />or within three (3) feet of the well head. <br />• Lack of or Inadequate Backflow Prevention. <br />(SjC Water Well Standards, Section 4,14. - Backflow Prevention) <br />Corrective Action: Install an approved protective device to prevont backflow and/or back <br />siphonage into a well, <br />• Sounding Tube or Air Vent not Sealed. <br />(SJC Water Well Standards, Section 4.13. - Sounding TubejAir Vent Pipe) <br />Corrective Action: Sounding Tube or Fill Pipe -Install a watertight, threaded cap at the upper end of <br />the pipe. Air Vent -extend pipe 12 inches above with end inverted in a downward direction and <br />covered with 16 -mesh screening. <br />• Excessive Vegetation or debris around Wellhead. <br />(1.S,JC Ord.
