<br />Geocon Project No. S2170-07-12 - 20 - June 6, 2022 <br />7.0 EMERGENCY RESPONSE PROCEDURES <br />7.1 Physical Injury <br />In the event of an accident resulting in physical injury, call emergency service personnel immediately <br />and perform first aid commensurate with training and seriousness of the injury. Severely injured <br />personnel are to be transported only by emergency service personnel and/or by ambulance personnel, <br />unless a life-threatening condition is judged to exist that must be addressed immediately. At the <br />hospital, a physician's attention is mandatory regardless of how serious the injury appears. <br /> <br />The PM is to be notified by the PSO, as soon after the injury as practical, regarding the nature of the <br />accident. A written report is also to be prepared and submitted by the PSO to the PM within 24 hours <br />of the accident. If the PSO is unable to make the report (due to injury) an individual designated by the <br />PM shall make the report. <br />7.2 Catastrophic Event <br />In the event of a catastrophic event (e.g., severe personal injury, fire, explosion, and/or property <br />damage), notify the fire/safety and rescue department immediately by dialing 911. <br /> <br />Any accident involving serious injury will require suspension of site activities until the PM (or <br />designee) has completed a review of the events and site conditions and authorized work to resume. <br /> <br />The PM (or designee) will notify the nearest Cal/OSHA District Office immediately (within 8 hours) <br />by phone or fax upon learning of a death or serious injury: <br />Modesto District Office <br />4206 Technology Drive, Ste. 3 <br />Modesto, California 95356 <br />Tel: 209-545-7310 <br />Fax: 209-545-7313 <br />Email: DOSHMOD@dir.ca.gov <br />7.3 Emergency Telephone Numbers <br />Fire/Police/Medical Assistance: 911 <br />Stockton Police Department: 209-937-8377 – Non-Emergency <br />Poison Control: 800-222-1222 <br />Other phone numbers may be available or required for emergency response at specific sites.
