Notification Requirements <br /> Directly after"Care of the Employee," make the following notifications, in order: <br /> • Contact the PM and HSM immediately <br /> • Contact the APTIM Incident Hot Line within 1 hour at(866) 299-3445 <br /> • PM will contact upper line management <br /> — The HSM will facilitate the incident investigation <br /> — Incident will be entered into APTIM Incident Management (AIM) system within the <br /> appropriate reporting time frames (See AMS-710-05-PR-02200). <br /> — Site supervisor will complete appropriate accident reports (see Appendix C) in <br /> accordance with AMS-710-05-PR-02300. <br /> All client requirements will be adhered to including pertinent personal injury incident reporting. <br /> Incident Other Than Personal Injury <br /> All incidents including fire, explosion, property damage, environmental release etc. will be responded in <br /> accordance with the site-specific Health and Safety Plan (SSHASP -see Section 9.0). In general, this <br /> includes securing the site appropriate to the incident, turning control over to the emergency <br /> responders, or securing the site and summoning appropriate remedial personnel or equipment. APTIM <br /> will immediately notify the client of any major incident, fire, equipment/property damage, and <br /> environmental incident with a preliminary report. A full report will be provided within the requirements <br /> in AMS-710-05-PR-02200. <br /> Motor Vehicle Incidents <br /> All motor vehicle accidents will be reported to the Help Desk, Project Management, and HSM. HSM will <br /> determine whether the motor vehicle incident requires the driver of the vehicle to obtain a drug and <br /> alcohol test. In general, chargeable, at fault accidents will require the operator to submit to a drug and <br /> alcohol screening. Special note regarding any incident: Should you become involved in any incident <br /> always contact the Regional H&S Manager for guidance. <br /> 2-8 <br />
