KLEINFELDER <br /> 2. GROUNDWATER MONITORING WELL DECOMMISSIONING <br /> In San Joaquin County a well destruction permit also is required prior to destroying a well. The <br /> appropriate well destruction permit, fees, and other required documentation, such as a copy of the <br /> work plan, will be submitted to the San Joaquin County Public Health Service Department <br /> Environmental Health Division (PHS/EHD). Upon approval of the well destruction permit <br /> application, wells will be destroyed as discussed below. <br /> 2.1 WELL DECOMMISSIONING <br /> The following describes how monitoring wells will be decommissioned at Forward Landfill. <br /> Field preparation activities will be followed as described in items 1.1 and 1.3 above. <br /> • g <br /> InitiallY prepare, tofor well decommissioning, the well head and cover will be removed <br /> from the well. <br /> • Destruction activities will be performed using a truck mounted drill rig equipped with a <br /> suitable length of tremmie hose. <br /> ' • The truck mounted drill rig will drill out the 4-inch diameter PVC casing with a 10-inch <br /> hollow stem auger. <br /> • Cement grout containing no more than 4% bentonite will be pumped through the hollow <br /> stem auger to the bottom of the well using tremie pipe. <br /> • The well will be grouted up with in 5 feet of the ground surface. <br /> • The remaining five feet will be filled with soil and compacted. <br /> 23-483849-G00/2310R027 Page A-2 January 19,2000 <br /> Copyright 2000 Kleinfelder,Inc. <br />
