SAN J OA U I Environmental Health Department <br /> COUNTY <br /> — <br /> Hazardous Materials Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> Satellite Dialysis University Park 590 E Harding Way, Stockton October 20, 2022 <br /> Violations <br /> 1055 1 See below Unlisted Abandonment/Illegal Disposal/Unauthorized Treatment-Local Ordinance ❑V ❑R ❑COS <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 5 HSC 25505(a)(1) Failed to electronically submit hazardous materials inventory for all reportable materials on site. <br /> OBSERVATION: About 1,372 pounds of dry bicarbonate solid and 147 gallons of bicarbonate solution were found <br /> inside the facility and have not been reported to CERS. <br /> REGULATION GUIDANCE: Any material that meets or exceeds the reportable quantity shall be reported in the <br /> facility's business plan. <br /> CORRECTIVE ACTIONS: Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/, add the materials to the hazardous material inventory, and submit to the EHD for <br /> approval. <br /> This is a Class II violation. <br /> 6 HSC 25505(a)(2) Failed to electronically submit a site map with all required content. <br /> OBSERVATION: The site map is not complete. The site map does not show/address the following required <br /> elements: <br /> 1. North orientation. <br /> 2. Loading areas. <br /> 3. Internal roads. <br /> 4. Adjacent streets. <br /> 5. Storm and sewer drains. <br /> 6. Access and exit point for structures and from the street. <br /> 7. Evacuation staging area. <br /> 8. Emergency response equipment (fire extinguishers and spill control). <br /> 9. Emergency shutoffs. <br /> 10. The location of the dry bicarbonate and the barcarbonae water mixture. <br /> 11.
