Hi Jeff, Attached is a copy of the older permit that we discussed previously and a map showing the locations for the new scope of work (locations are highlighted yellow). We would like the permit to be amended to reflect the new scope of work that will be taking place December 14th to 16th. All entities (location, consultant, driller, etc) remain the same, the addendum for the new work includes the activities listed below. Please let me if you require anything else in order to issue an updated permit for this new scope of work, thank you. • 7 x CIP borings; and • 4 x vertical soil borings. But in brief, you would need to provide to the EHD the following: 1. Completed Master File Record, especially the billable party information.This will also act as a service request to review your work plan/ proposal 2. Completed Site Mitigation Well/boring application for each separate parcel (APN) 3. Work Plan complete with map of where the borings/well are to be installed, and construction diagram of any proposed monitoring wells 4. If this work is required by a lead agency (i.e. EPA, RWQCB, DTSC, etc.), documentation that they are approving this proposed work 5. Access agreement if the work is being done on a property that does not belong to your client 6. Fees. • $456 for the plan check fee which covers 3 hours (if it's a large or complicated plan, it may require $760 which covers 5 hours) • Boring Permit fees averages $456 ($152/hour) perap rcel (each parcel requires a separate permit), which buys 3 hours of inspection time. Please be aware that if the inspection goes over the 3 hours which includes drive time, the time over the 3 hours will be billed to the billable party. Hence there are occasions where more than 3 hours is submitted to avoid being billed at a later time. Please feel free to
