But in brief, you would <br /> need to provide to the EHD the following: <br /> 1. Completed Master File Record, especially the billable party information.This will also act as a <br /> service request to review your work plan/ proposal <br /> 2. Completed Site Mitigation Well/boring application for each separate parcel (APN) <br /> 3. Work Plan complete with map of where the borings/well are to be installed, and construction <br /> diagram of any proposed monitoring wells <br /> 4. If this work is required by a lead agency (i.e. EPA, RWQCB, DTSC, etc.), documentation that <br /> they are approving this proposed work <br /> 5. Access agreement if the work is being done on a property that does not belong to your client <br /> 6. Fees. <br /> • $456 for the plan check fee which covers 3 hours (if it's a large or complicated plan, <br /> it may require $760 which covers 5 hours) <br /> • Boring Permit fees averages $456 ($152/hour) perap rcel (each parcel requires a <br /> separate permit), which buys 3 hours of inspection time. Please be aware that if the <br /> inspection goes over the 3 hours which includes drive time, the time over the 3 <br /> hours will be billed to the billable party.
