PES Environmental, Inc. <br /> 1.0 INTRODUCTION <br /> This Health and Safety Plan (HSP) has been prepared by PES Environmental, Inc. (PES) <br /> and describes the minimum health and safety requirements for planned field activities by <br /> PES team members (i.e., PES staff and subcontractors) at the 8095 Rio Blanco Road site <br /> (the site) located in Stockton, California. The planned field activities at the site consist of the <br /> removal of petroleum hydrocarbon impacted water from previously installed wells. Removal <br /> and disposal of hydrocarbon impacted water will be completed by a vacuum truck and hand <br /> removed by PES trained staff. <br /> In addition to the procedures and requirements described in this HSP, all on-site PES personnel <br /> shall follow applicable procedures and requirements specified in the PES Field Manual for <br /> Soil and Groundwater Investigations and by Federal, State, and local authorities. As specified <br /> in Title 29 CFR 1910.120, this HSP has been prepared to address the basic requirements of <br /> the overall safety and health program, with attention to those characteristics of site-specific <br /> activities. Any modifications made to this HSP because of encountered field conditions must <br /> be approved by the site-safety officer (SSO) and/or project manager (PM). <br /> A copy of this HSP will be available at the site during all activities. <br /> 2.0 KEY PERSONNEL AND RESPONSIBILITIES <br /> 2.1 Key Personnel and Qualifications <br /> The PES PM is James P. Dunn. The SSO is Gavin M. Creps, or a designated alternate <br /> engineer or geologist. The project field staff have completed 40 hours of comprehensive health <br /> and safety training, which meets the requirements of Title 29 CFR 1910.120. In addition, <br /> the PM and the SSO have completed an 8-hour supervisory course in health and safety <br /> management, including training in site safety planning, emergency planning, and sampling <br /> safety. The PES corporate Health and Safety Officer (HSO) is Bill Frizzell. Documentation <br /> for all training completed by PES personnel is available from the HSO on request. <br /> 2.2 Responsibilities <br /> The PM is responsible for the preparation and review of this HSP for accuracy and <br /> incorporating new information or guidelines, which aid the SSO in further definition and <br /> control of the potential health and safety hazards associated with the project. The PM and <br /> SSO are responsible for assuring that adequate training and safety briefing(s) for the project <br /> are provided to the project team. The PM or SSO will provide a copy of this HSP to each <br /> member of the project field team. The HSO is responsible for coordinating the PES corporate <br /> health and safety program.
