SOP-004 Standard Operating Procedure Chain of Custody <br />Page 1 of 3 Revision No. 1 April 2019 <br /> <br />Sharpe Army Depot <br />1.0 Purpose <br />The purpose of this standard operating procedure (SOP) is to delineate protocols for use of the chain-of-custody (COC) form. <br />2.0 Scope <br />This SOP applies to all personnel responsible for collecting, shipping and analyzing environmental samples. <br />3.0 Method <br />3.1 General <br />COC forms are used to legally track samples from time of collection through completion of laboratory analysis. <br />3.2 Procedures <br />The following information will be preprinted on the COC form when possible: <br />• Project name; <br />• Name and address of laboratory; and <br />• Potential analysis and method numbers. <br />The following information will be written on the COC form by the sample controller/shipper: <br />• Site name; <br />• Name of receiving laboratory; <br />• Sample IDs for all samples in a particular cooler/shipping container; <br />• Sample matrix or matrix code (e.g., SO for soil); <br />• Sample type (environmental, trip blank, equipment blank, etc.); <br />• Analysis requested by method number unless other arrangements are made with the receiving laboratory; <br />• Number of containers; <br />• Quality Control (QC) required (to indicate the sample is to be used for matrix spike/matrix spike duplicate analyses); <br />• Date of collection (mm/dd/yy or m/d/yy: 04/03/18 or 4/3/98 is April 3, 2018); <br />• Time of collection (military format); <br />• Signature of individual who prepares the COC form; <br />• Cooler identification (ID); <br />• Carrier service and airbill number; and <br />• Signature of individual relinquishing samples along with the date and time of relinquishment. <br />Upon completion of the form, retain two copies and affix the original and one copy to the inside of the sample cooler (in a Ziploc® bag to protect from moisture), to be sent to the designated laboratory.
