Tom:
I do not believe there are any payment made in what you’d provided. The EHD cannot review or
process anything until the moneys are submitted. Please take a look at the guidance below:
Please visit our website, and look under Forms:
https://www.sjgov.org/department/envhealth/forms
Click on Site Mitigation tab, there you’ll find all the permit forms you would need for your Phase II
Site Assessment or other Site Mitigation endeavor, as well as a guidance. But in brief, you would
need to provide to the EHD the following:
1. Completed Master File Record, especially the billable party information. This will also act as a
service request to review your work plan/ proposal
2. Completed Site Mitigation Well/boring permit application for each separate parcel (APN).
Please be aware well boring permits are only issued to C-57 licensed holder per California
requirement, and therefore requires either their signature on the permit application, or a
person authorized by the C-57 licensed holder on the attached License Contractor Declaration
form (see bottom of the form).
3. Work Plan complete with map of where the borings/ well are to be installed, and construction
diagram of any proposed monitoring wells. Must include a little bit of
background/history/future developmental goal (can be very brief: who, what, when, where,
why. A short paragraph or two will work, more if required), and the proposed analytical
testing.
4. If this work is required by a lead agency (i.e. EPA, RWQCB, DTSC, etc.), their approval
documentation for this proposed work
5. Access agreement (or encroachement, revocable permits if in city/county/government Right
of Way) if the work is being done on a property that does not belong to your client
6. Fees.
- $468 for the plan check fee which covers 3 hours (if it’s a large or complicated plan, it
may require $780 which covers 5 hours)
- Boring Permit fees averages $468 ($156/hour) per parcel (each parcel requires a
separate permit), which buys 3 hours of inspection time. Please be aware that if
the inspection goes over the 3 hours which includes drive time, the time over the 3
hours will be billed to the billable party. Hence there are occasions where more
than 3 hours is submitted to avoid being billed at a later time. Please feel free to
contact me for variations of the fees required.
- Well Permit fees: $156 per well (not parcel) per parcel, but the fees maxes out at 5
wells per parcel.
7. Possible other documents (i.e. if Corps of Engineering is involved)
The EHD has 20 business days to issue the permit or respond to the request after the fees have been
submitted. An expedited process is also available at 1.5x fee rate for the work plan review, which
EHD will respond the the request or provide the permit within 3 business days.
You can submit all the above to ehsitemitigation@sjgov.org, or by mail. Payment can also be made
on our website:
https://www.sjgov.org/department/envhealth/
Best Regards,
Jeffrey Wong
From: Fortner, Thomas <Thomas.Fortner@WestonSolutions.com>
