SAN J 0 A Q U 1 N Environmental Health Department <br /> u ; i y 1 Iii AMENDED <br /> 12/5/2022 <br /> Hazardous Materials Program Inspection Report <br /> Facility Name: Facility Address: FDat,-.AMAZON.COM SERVICES LLC-SCK3 3565 N AIRPORT WAY, MANTECA ber 31, 2022 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 11 HSC 25505(a)(4) Failed to provide or document initial and/or refresher training to appropriate personnel. <br /> OBSERVATION: Facility personnel could not provide hazardous materials safety training records for the past three <br /> years that include review of the site specific Consolidated Emergency Response/Contingency Plan uploaded to the <br /> California Environmental Reporting System (CERS). <br /> REGULATION GUIDANCE: The business plan shall include provisions for ensuring that appropriate personnel <br /> receive initial and annual refresher training.All employees shall be trained in safety procedures in the event of a <br /> release or threatened release of a hazardous material, including, but not limited to,familiarity with the following <br /> plans and procedures: <br /> (A) Immediate notification contacts to the appropriate local emergency response personnel and to the unified <br /> program agency. <br /> (B)Procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br /> persons, property, or the environment. <br /> (C) Evacuation plans and procedures, including immediate notice,for the business site. <br /> This training shall be documented electronically or by hard copy and shall be made available for a minimum of three <br /> years. <br /> CORRECTIVE ACTION: Immediately provide employee training for appropriate personnel and submit a copy of the <br /> training records to the EHD. <br /> This is a repeat violation, Class II. <br /> 12 HSC 25505(c)Failed to have the HMBP readily available to facility or EHD personnel. <br /> OBSERVATION: Employees do not have on-site access to the Emergency Response plan, site map and the <br /> inventory information. This information is uploaded to CERS. Only upper management have access to this <br /> information. <br /> REGULATION GUIDANCE: The emergency response plans and procedures,the inventory of information required <br /> by this article, and the site map required by this section shall be readily available to personnel of the business or the <br /> unified program facility with responsibilities for emergency response or training pursuant to this section. <br /> CORRECTIVE ACTION: Immediately make arrangements to have the business plan readily available for review. <br /> This is a repeat violation, Class II. <br /> Overall Inspection Comments: <br /> An inspection checklist was provided to the facility operator on the day of inspection.
