ONE 14 YEAR <br />OLD ATE CARNITAS TACOS WITH GUACAMOLE. THE 14 YEAR OLD DEVELOPED SYMPTOMS ON 8/7. ON 8/11 WAS <br />DIAGNOSED WITH SHIGA TOXIN-PRODUCING E-COLI. NO ONE ELSE IN THE PARTY GOT SICK. <br />VIOLATIONS AND CORRECTIVE ACTIONS <br />Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7; <br />113700. All violations must be corrected within specified timeframe. Violations that are classified as "MAJOR" pose an immediate threat to public health <br />and have the potential to cause foodbome illness. All major violations must be corrected immediately. Non-compliance may warrant immediate closure of <br />the food facility. <br />OVERALL INSPECTION NOTES AND COMMENTS <br />FOOD ITEM -- LOCATION --TEMP ° F -- COMMENTS <br />No Temperature Data Collected <br />NOTES <br />I went to the facility and spoke with the manager who was not aware of this complaint prior to this investigation. <br />There have been no ill employees for the last two weeks. <br />There is a total of two restrooms. <br />Per the manager, one restroom was closed (the battery for the hand wash sink sensor was out) and one remained open. <br />The restroom that was closed was closed for 2 days. <br />Warm water at the hand sink at the restroom near the register is at 101F. <br />Warm water at hand sink restroom near the entrance door is at 96F. <br />Provide warm water at this sink between 100E-108F, if on a non adjustable faucet, not to exceed 108F. <br />Carnitas taco is carnitas put in a soft tortilla or in a hard tortilla and then salsa, beans, rice, cheese, lettuce and guacamole. <br />Carnitas come to the facility pre cooked and pre packaged and are stored in the walk-in refrigerator. <br />I observed pre-cooked and pre-packaged carnitas in the walk-in (41F).Carnitas are taken from walk-in and placed and cooked <br />until temperature is 165F or higher and then placed on the front hot assembly line (203F) or on the digital take out table line <br />(185F) <br />Per manager guacamole is prepared by taking avocados from walk-in and washing in Victory wash and then placing in "hotel" <br />pans and placing back into the walk-in. <br />When prepared avocados are peeled, scooped and pitted macerated with lemon and lime juice, and smashed and cilantro <br />and onion is added. <br />I observed carnitas taco assembled, I observed proper hand washing and utensil use. <br />Hand sinks in kitchen are stocked with soap and paper towels. <br />Hand sinks in restrooms have functional soap dispensers and hand dryers. <br />FA0022577 C00055137 SC004 08/16/2022 <br />EHD 16-23 Rev. 06/30/15 <br />Page 1 of 2 Food Program Complaint Inspection Report <br />1868 E.
