Provide adequate traffic control. <br />7. AC T27CCR20860 Trucks were driven in excess of 15 miles per hour. <br />Michael Kith <br />a. Ensure that all trucks obey the posted speed limit. <br />8. AC T27CCR20870 Load checking program does not address the removal of prohibited waste. <br />a. Revise the load checking program to include the removal of all prohibited waste. <br />b. Ensure that all hazardous waste and unacceptable waste are removed. <br />9. AC T27CCR20919 The shallow probe of GPI I and the shallow and middle probes of GP12 might be plugged. <br />There was a long air influx into those probes after gas is sampled. <br />a. Provide functional gas probe/sample port at all property boundary. <br />b. Ensure that those probes were not plugged or clogged. Submit finding(s) to EHD. <br />10. Violation T27CCR20919.5 The concentrations of methane gas were as following: <br />In GP9: 49.50 % in shallow 33.90 % in middle 0.00% in deep <br />In GP12: 5.20 % in shallow 0.00 % in middle 8.10 % in deep <br />a. Ensure that the concentration of explosive gas at the property boundary is below 5 % by volume. <br />b. Follow directions listed in T27CCR20919.5(c) and in the Amended Notice and Order 03-01. <br />c. Also see previous inspection report for the previous violation of this section. <br />11. AC T27CCR20686 Excessive amount of waste was listed as beneficial reuse. For the week of 10/17/05, <br />31,443.22 tons of waste was listed as disposal, and 22,200.59 tons was listed as beneficial reuse. <br />The waste to cover ratio for that same period was 1.4 to 1. <br />a. Provide justification for such excessive usage of the cover and other beneficial reuse to LEA and the Board. <br />b. Also see inspection dated 4/20/05, 5/25/0, 6/24/05, 7/27/05 and 9/29/05 for previous area of concern of this section. <br />12. Notes: <br />A.
