Continued: <br />RECEIVED BY <br />OPERATOR <br />b. The increase in the tonnage received at this facility constitutes a significant change in the operation. All significant <br />change in operation requires prior approval from the LEA and concurrence from the Board. <br />c. Also see inspection report dated 4/20/05, 5/25/05, 6/24/05, 7/27/05, 8/26/05, 9/29/05 and 10/28/05 for previous <br />violation of this section. <br />3. Violation PRC 44014(b) The tonnage of waste received/accepted at this facility exceeded the permitted <br />maximum tonnage of 46,080 tons per week, not exceeding 8668 tons per day. On November 2 and 3, this facility <br />received 9,465.65 and 9,419.58 tons of waste respectively. <br />a. The amount of waste received at this facility shall not exceed the permitted maximum tonnage of <br />46,080 tons per week, not exceeding 8,668 tons per day. <br />b. Also see inspection dated 4/20/05, 5/25/05, 6/24/05, 7/27/05, 8/26/05, 9/2905 and 10/28/05for previous violation of <br />this section. <br />5. Violation T27CCR21600 RDSI has not been amended to include information listed in <br />T27CCR21600(b)(6)(B), the ADC usage and waste to cover ratio. <br />a. Submit application to amend RDSI to include the above information to LEA. <br />b. Also see inspection dated 4/20/05, 5/25/05, 6/24/05, 7/27/05, 8/26/05, 9/29/05 and 10/28/05 for previous violation <br />of this section. <br />c. The proposed JTD amendment received on August 2nd, 2005 was rejected by this LEA in a response letter dated <br />August 23`d, 2005 due to conflicting with existing approvals for the facility. <br />6. AC T27CCR20690 The green waste destined for ADC usage has lots of household waste in it. <br />a. Remove all contaminants from the green waste destined for ADC. <br />8. AC T27CCR20870 7 waste tires found in the waste. The tires were removed after the LEA alerted the operator. <br />a. Provide the load checking program to include the removal of all prohibited waste. <br />b. Ensure that all hazardous waste and unacceptable waste are removed. <br />9. AC T27CCR20919 The shallow probe of GPI I and the shallow and middle probes of GP12 might be plugged. <br />There was a long air 'nflux into those probes after gas is sampled. <br />INSPECTOR SIGNATURE: DATE: <br />
