Decontamination and sterilization areas shall be separated from procedure areas by a space of at least 5 feet or by a <br />cleanable barrier and equipped with a sink with hot and cold running water, containerized liquid soap, and single use paper <br />towels dispensed from a wall-mounted, touchless dispenser that is readily accessible to practitioner. HSC 119314(c) — The <br />sterilization room was not supplied with containerized liquid soap. Ensure there is soap in the sterilization room. <br />11. The procedure area shall be equipped with a sink with hot and cold running water, containerized liquid soap, and single use <br />paper towels dispensed from a wall-mounted, touchless dispenser that is readily accessible to practitioner. HSC 119314(b) — <br />The soap dispenser, in the yellow room, was empty. Ensure the soap dispenser has soap, at all times. <br />18. The facilities client consent form and medical questionnaire must include all of the information listed in Form B, and are <br />available for inspection. HSC 119303— A client record was missing a parent signature and some records did not have the <br />lot number of the needle(s) used. Ensure all client records are signed by the parent(s) (for minors only) and lot numbers <br />for the needles are written or the package is attached to the form. <br />A body art facility shall be separated from any residential areas used for sleeping, bathing, or meal preparation and shall not <br />share a common entrance or toilet facility with residence. The toilet facility must have a sink with hot and cold water, <br />containerized liquid soap, and single-use paper towels that are dispensed from a wall-mounted, touchless dispenser. HSC <br />119314— The restroom sink did not have hot water. Ensure there hot water is supplied in the restroom. A reinspection <br />will be conducted to check for hot water. <br />A body art facility shall have floors, walls, and ceilings that are smooth, nonabsorbent, free of open holes, and washable. <br />The body art facility shall be free of insect and rodent infestation. The procedure area must be separated, by a wall, ceiling to <br />floor, from nail and hair activities and be separated from all business not related to body art. Approval of co-located body art <br />facilities are at the discretion of the LEA. A procedure area must be equipped with adequate light to perform a procedure. Each <br />procedure area shall have lined waste containers. HSC 119314 & 119317— The following was observed: <br />Restroom — Holes on the wall. <br />Blue Room — Holes near the sink. <br />Purple Room — Holes in wall and the area behind the table needs to be painted. <br />Green Room — There are holes in the wall under the TV. The floor and baseboard are in need of repair, next to the <br />cabinet. <br />Paint the purple room (as needed) and repair all holes and floor/ baseboard(s). Provide EHD photographic evidence of <br />corrections. <br />Unless otherwise noted, correct all violations and provide evidence of corrections to <br />the END by October 1, 2022. <br />Reinspection on/about: A reinspection fee of $156 per hour may be charged.
