Complaint Investigation Summary Report#:5105 COMPLAINT ID: CO0046940 Site Location: 15890 THIRD ST Activity Summary Activity Date Recorded by 08/22/2018 Went to site with with Lee Myers and Bonnie Lew-DTSC and Kelly McDaniel SJC HENRY Deputy District Attorney. Met with Jeff Camarena, sob of property owner. Jesse Camarena too sick to show us around. Walked with Jeff to look at the open well. He said he wasnt sure how much the permit was to fix. I told him I would find out for him. ASked if he had received the firast notice to abate sent out by Naseem Ahmed July 16, 2018. He said he was not aware of it but his dad may have got it. I issued 2nd notice to abate drafted by Naseem, advising to correct the violations immediately. He said ok. We then walked through the back yard. It appears large piles have moved, possibly contaminated dirt is gone. I issued NTA to cease the discharge of hazardous waste to the ground and cease transportation of hazardous waste by unlicensed hauler to unpermitted facility. Samples taken by DTSC June 12, 2018 and May 4, 2018 indicate hazardous waste. It was explained to Jeff the concerns we all had about him and potentially others transferring hazardous waste from the backyard to other areas. He said he understood. Jeff was told of the need for clean-up but it needed to be done by a licensed person, trained to handle hazardous waste. I explained the voluntary clean-up program and told him I would get more information for him. He is aware no further clean-up can take place at this time until a plan is made. 08/23/2018 Printed pictures, tried to email pictures and nr,tir.P to abates to to Jeff Camarena. HENRY The email address he provided _ )m did not work. Called Jeff at to verify email address, left voicemail. 09/25/2018 research well and septic permits, history for water sampling event tomorrow HENRY 09/26/2018 Went to site, met with DTSC Lee Myers, Bonnie Lew and SWRCB Mark Bare. HENRY Mark Bare took water samples from well to see if water has been contaminted by the CRT processing. per Mark the samples should be back within 10-14 days. Jeff Camarena gave us permission to take the sample and look around the back to see if much had changed. It did not appear that any further clean-up had happened. jeff said he did have a well permit. I told him I would research in the office because I did not see an SR. 10/01/2018 Worked with Naseem to find a permit for this site. Per Naseem the site has a well HENRY permit for cross connection. It was determined they did not need a pump permit . WP0038696 01/10/2019 Called Jeff phone number and left his voice mail. Under permit AHMED Wp0038696 two of the three violations have been fixed. Sample tap in not adequate. Waiting for Jeff response. 02/28/2019 Called and got hold of Jeff Camarena. Jeff understood that current AHMED sample tap needs to be non-threaded and need to be between the well head and the check valve or within 3 feet of the well head. 06/07/2019 Jeff Camarena sent me photos of installed sample tap. I called Jeff to speak with AHMED him about the sample tap what he needs to adjust however his phone number is disconnected. I emailed him to call me back. 07/19/2019 Called Jeff and left him voice mail about the sample tap not being adequate. AHMED Waiting for Jeff response. 5105.rpt Page 2 of 3
