Weber Ave., Third Floor <br />Stockton, CA 95202 <br />Dear Kasey, <br />As follow up to the Medical Waste Management Program review for Lodi Memorial Hospital and Lodi <br />Memorial Hospital West on December 11, 2000, 1 am happy to provide the following information <br />regarding items discussed. <br />BIOHAZARDOUS BAG'DART' TEST COMPLIANCE REPORT - This particular issue has resulted in a <br />major ordeal for the manufacturer of the bags currently in use. I have obtained documentation from <br />that provider and am including a copy of such for your review. <br />Due to the complications involved in securing the required documentation, while attempting to seek <br />an acceptable level of quality in that product, it has been decided that Lodi Memorial Hospital must <br />move toward full vendor replacement regarding biohazardous bags and has temporarily obtained an <br />alternate product. <br />Today I met with a local provider who has been working feverishly with both Mr. Jack McGuirk and <br />Mr. Mike Schott regarding problems identified with our current product line. It appears that Lodi <br />Memorial Hospital has been selected to receive a newly improved product developed under the <br />guidelines provided by Mr. McGuirk. The new liner or "Bio Bag" and associated compliance <br />documentation is scheduled for delivery early next week. I plan to maintain close monitoring and <br />evaluation of this liner until expected results are proven or another product is secured. <br />2. PROPER LABELING FOR CONTAINERS - As identified during the review, some areas of the facilities <br />were found with missing or minimal labeling on biohazardous waste containers. Immediate <br />correction has been made with additional labels affixed to containers located in the following areas: <br />• Clinical Lab (located in the Conrad Building near the main hospital, <br />• ER Triage area on first floor of the main hospital building, <br />• Radiology area X -Ray rooms on first floor of the main hospital building, <br />• 2W Medical Surgical patient area located on the second floor of the main hospital building, <br />• LMHW patient room where new container was found in use and lacking all required labeling. <br />If you have questions regarding these corrective measures please contact me to discuss them further.
