Fairmont Ave. <br />PO Box 3004 <br />Lodi, CA 95241-1908 <br />Re: Biological Indicators for Steam Sterilizer <br />Ms. Fulton, <br />Please consider this an addendum to the medical waste inspection report I provided to you on <br />September 4, 1996. Ms. Campbell provided the Bacillus stearothermophilus vials presumably <br />for the past year however I was unable to read any dates and unable to decipher which were <br />the controls and which were the actual tests. In addition, I was unable to determine whether <br />or not your sterilizer had attained adequate sterilization conditions. Please provide to me, in <br />writing, who will be responsible to correct this violation of Health and Safety Code section <br />118215 (c)(4) and (c)(5), within 15 days.
