According to Daniel Stafford, <br />the Secondary Emergency Contact, Jason Daubman has not been affiliated with this facility for at least four or five <br />months. <br />REGULATION GUIDANCE: This information must be current and submitted as part of the business plan. <br />CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calep;a.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. <br />This is a Class II violation. <br />5 HSC 25505(a)(1) Failed to electronically submit hazardous materials inventory for all reportable materials on site. <br />1. OBSERVATION: The 2,440 cubic feet of carbon dioxide was removed from the site about 1 year ago and the <br />material has not been removed from the chemical inventory. <br />REGULATION GUIDANCE: Any material that meets or exceeds the reportable quantity shall be reported in the <br />facility's business plan. <br />CORRECTIVE ACTIONS: Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/, add the materials to the hazardous material inventory, and submit to the EHD for <br />approval. <br />…………………………………………………………………………………………………………………… <br />2.
