The Facility Employee Training <br />Certificate shall contain accurate information for all employees trained. <br />CORRECTIVE ACTION: Update the Facility Employee Training Certificate to contain the accurate information and <br />submit a copy of the updated form to the EHD. <br />This is a repeat violation, Class II. <br />303 <br />CCR 2630(d), 2641(a) Failure to maintain piping to allow liquid to flow into sump (i.e. failure to remove test boots). <br />OBSERVATION: Diesel STP test boot was not pulled back and Schrader valve was plugged. Any potential release <br />from the primary containment into the secondary containment in the Diesel piping does not flow into a collection <br />sump. <br />REGULATION GUIDANCE: All secondary containment systems shall be sloped so that all releases will flow to a <br />collection sump located at the low point of the underground piping. <br />CORRECTIVE ACTION: Immediately contact a properly licensed, trained, and certified contractor to ensure that any <br />releases in the Diesel piping flows into the Diesel STP sump. <br />This is a Class II violation. <br />308 <br />CCR 2632(c), 2634(d), 2636(f) Failure of the leak detection equipment to have an audible and visual alarm. <br />OBSERVATION: The float and chain in UDC 7/8 (Cardlock side) failed to stop the flow of product at the dispenser <br />when tested. <br />REGULATION GUIDANCE: All monitoring equipment shall be maintained to activate an audible and visual alarm or <br />stop the flow of product at the dispenser when it detects a leak. <br />CORRECTIVE ACTION: The service technician adjusted the float and chain and verified functionality. <br />This is a repeat violation, Class II. <br />FA0002347 PR0231761 SCO01 10/03/2023 <br />EHD 23-01 Rev. 12/06/2021 Page 6 of 7 Underground Storage Tank Program OIR <br />1868 E.
