Designated operator inspection reports on or after October 1, 2018 shall be retained on site <br /> for thirty-six months. <br /> CORRECTIVE ACTION: Locate and ensure that copies of the previous thirty-six months of designated operator <br /> inspection reports are maintained on site. <br /> This is a repeat violation, Class Il. <br /> 210 CCR 2715(c)Failure to provide initial training,train every 12 months, and have a trained employee present. <br /> OBSERVATION: The designated operator employee training for Sam Mohammed was not performed and he was <br /> the only employee on site. <br /> REGULATION GUIDANCE: The designated operator shall train facility employees in the proper operation and <br /> maintenance of the UST system at least once every 12 months. The training shall be conducted before the <br /> individual performs the duties of a facility employees.The training shall include, but is not limited to: <br /> 1. Operation of the UST system in a manner consistent with the facility's best management practices <br /> 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br /> 3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br /> 4. Name of the contact person(s)for emergencies and monitoring equipment alarms <br /> Initial training shall also include a site-specific practical demonstration that provides both sharing information and <br /> evaluating knowledge for responding to emergencies, leak detection, monitoring alarms, spills and overfills. At least <br /> one of the facility employees present during operating hours shall have current training. <br /> CORRECTIVE ACTION: Ensure that employees have been trained by the designated operator, maintain the list on <br /> site, and submit a copy of the training records to the EHD. <br /> This is a repeat violation, Class Il. <br /> 308 CCR 2632(c), 2634(d), 2636(f) Failure of the leak detection equipment to have an audible and visual alarm. <br /> OBSERVATION: The float and chain in the 3/4 under dispenser containment sump failed to stop the flow of product <br /> at the dispenser when tested. <br /> REGULATION GUIDANCE:All monitoring equipment shall be maintained to activate an audible and visual alarm or <br /> stop the flow of product at the dispenser when it detects a leak. <br /> CORRECTIVE ACTION: The service technician adjusted the float and chain and verified functionality. <br /> This is a repeat violation, Class ll. <br /> Overall Inspection Comments: <br /> FA0006185 PR0504388 SCO01 07/31/2023 <br /> EHD 23-01 Rev.12/06/2021 Page 5 of 6 Underground Storage Tank Program OIR <br /> 1868 E.
