OBSERVATION: The monitoring plan is not current and/or not approved by the EHD. -The name of the first person having responsibility is not current. General Manager, William Shaw is no longer with the facility. REGULATION GUIDANCE: The monitoring plan must be uploaded to the California Environmental Reporting System (CERS). CORRECTIVE ACTION: Immediately log into CERS, make the necessary changes, and submit for review by the EHD. This is a minor violation. 208 CCR 2716(a-e) Failure to comply with the designated operator inspection requirements. OBSERVATION: The 4/20/2022 designated operator report was not signed by the owner/operator. REGULATORY GUIDANCE:Within 48 hours of the completion of the designated UST operator visual inspection, the designated operator shall sign and provide the owner or operator with a copy of the inspection report. Within 72 hours of being provided a signed copy of the report,the owner or operator shall provide a description of each corrective action taken or to be taken and shall sign and date the report, acknowledging the identified compliance issues. CORRECTIVE ACTION: Ensure all designated operator inspection reports are completed and signed within the required time frame in the future. This is a repeat violation, Class II. 209 CCR 2715(c)(3), 2715(c)(4) Failure to maintain a copy of the"Facility Employee Training Certificate"on-site. OBSERVATION: The Facility Employee Training Certificates dated 4/12/2022, 8/10/2022,4/12/2023 and 4/26/2023 listed incorrect initial training and assumed responsibility dates. REGULATION GUIDANCE:All site employees must be trained prior to taking responsibility as a facility employee if after October 1, 2018, or within 30 days of hire if prior to October 1, 2018. The Facility Employee Training Certificate shall contain accurate information for all employees trained. CORRECTIVE ACTION: Update the Facility Employee Training Certificate to contain the accurate information and submit a copy of the updated form to the EHD. This is a minor violation.
