Weber Avenue, Third Floor <br />Stockton, CA 95202 <br />Dear Ms. Foley: <br />Provided is the information requested as a result of the inspection conductedon ;June 9, 2005 at the following locations: <br />975 S. Fairmont Avenue (Lodi Memorial Hospital) and 800 S. Lower Sacramento Road (Lodi Memorial Hospital West). <br />Lodi Memorial Hospital - 975 S. Fairmont Avenue, Lodi, CA 95240 <br />Findings: <br />Item 11. Medical Waste Management Plan is not current as Donna McCauley is,no longer the person responsible <br />for implementation of the MWMP. Action: Update plan and submit this to the Office by 7/10/05. <br />Response/Action: Lodi Memorial Hospital shall submit the necessary documentation indicating the change <br />in personnel responsible for the Medical Waste Management Programand plan: The position vacated by <br />Ms. Donna McCauley, Safety/Security Officer has been refilled effective June 13, 2005. The <br />documentation reflecting the change will be submitted by the indicated deadline. <br />Item 16. One load of med waste from the sterilizer did not appear to:mgpt time and temp requirements on 5/26/05. <br />Action: Follow-up with the load and submit to this Office by 6/17/05.. This is a repeat violation. <br />Response/Action: After an internal investigation conducted by Mr. Robert Hernandez, Environmental <br />Services Manager, the following was discovered. The 'sterilizer run that had no annotation was a no-load, <br />test run. The operator failed to indicate this by written notation on the recording wheel. The staff member <br />involved was re -instructed to indicate that the run was a NO LOAD, TEST along with the date it was <br />performed. <br />
