• <br /> c. Immediately use the required biohazard bag as specified in the Medical Waste <br /> Management Act, Chapter 2 Section 117630. <br /> d. The Biohazard bag must be used for all containerization of biohazardous waste except <br /> pharmaceutical waste. <br /> —► Provide Photo (chemo container with red biohazard liner) within 2 <br /> weeks. Completed and sent 6-16-11. <br /> 3. Section 118307—Interim storage labeled <br /> a. Intermediate storage areas shall be marked with the international biohazardous <br /> symbol or the signage described in Section 118310. These warning signs shall be <br /> readily legible from a distance of five feet. <br /> _+ Provide photos in OB Department showing the appropriate signage <br /> being used for all Interim storage areas within 2 weeks. Completed and <br /> sent 6-16-11. <br /> 4. Section 118275 (g)—Pharm label/storage <br /> a. Pharmaceutical Waste (Section 117635 (g) shall be segregated for storage and when <br /> placed in a container at secondary container,that container shall be labeled with the <br /> words "Incineration Only"on the lid and on the sides as to be visible from any lateral <br /> direction. Pharmaceutical waste containers at this facility do not meet the labeling <br /> requirements as specified in Chapter 9 Section 118275 (g). <br /> __+ Provide all the required labeling and place the labeling as specified <br /> above onto all pharmaceutical waste containers for the next routine <br /> inspection. Stericycle is working to provide a solution to this issue. <br /> Lodi Memorial Hospital West <br /> 1. Section 118280—MW label & container <br /> a. Containers shall be labeled with the words `Biohazardous Waste"or with the <br /> international biohazard symbol and the word"Biohazard"on the lid and on the sides <br /> so as to be visible from any lateral direction. <br /> b. Label all medical waste containers accordingly with the Medical Waste Management <br /> Act requirements sited in Chapter 9 Section 118275 and 118280. <br /> Biohazardous waste containers observed at the clinical facilities did not <br /> have all the required labeling. <br /> Place "Biohazard Waste" label on all clinic Biohazardous containers as <br /> required. Completed and sent 6-16-11. <br /> 2. Section 118307—Interim storage labeled <br /> a. Intermediate storage areas shall be marked with the international biohazardous symbol or <br /> the signage described in Section 118310. These warning signs shall be readily legible <br /> from a distance of five feet. <br /> Pharmaceutical waste containers and biohazardous sharps containers <br /> were found in a storage shed within the designated accumulation area. <br /> No signage identifying the storage shed as interim storage was seen at <br /> the time of the inspection. <br /> Apply the appropriate signage to all interim storage (pharmaceutical <br /> container inside designated accumulation areas) found at this facility. <br /> 2 <br />
