Third Floor <br />Stockton, CA 95202 <br />Please except the following as our response to the listed violations from your Routine <br />Inspection Report dated 6-20-02. <br />Violation: # 11) Update Med. Waste Mgmt plan to indicate the appropriate contact <br />person for medical waste. <br />• Please see attached Hazardous Materials and Waste Management plan, section <br />V, paragraph C, which states that the Facilities Manager is the person <br />responsible for the Hazardous Materials and Wastes Management Plan. <br />Violation: # 16a.) Steam Sterilization charts indicate time and temperature had not been <br />achieved in Feb 2002, November 2001 and several other times in 2001. You must <br />indicate that loads were re -run and achieved proper time and temperature or that the load <br />was hauled off by a registered Medical Waste hauler. Modify your policies \ Procedures <br />and train employees to implement these requirements. <br />• After review of this problem, it was noted that the temperature charts were <br />locked in a sealed case and employees were unable to note deficiencies with run <br />times or temperature problems due tot em being unable to reach the charts. We <br />will correct this problem by unlocking the chart box and allow access for <br />employees to be able to note deficiencies with runs on the charts themselves, or <br />we will post a clipboard in the sterilizer area and employees will be able to note <br />problems there. <br />Violation: 16b.) The biological indicator records indicate growth of the indicator without <br />documenting what was done to correct it.
