M E M 0 R A N D U M JUL 2 8 2004 <br />ENVIRONMENT HEALTH <br />PERMIT/SERVICES <br />TO: Kasey Foley, Environmental Health Department <br />FROM: Inez Aldridge, Director of Housekeeping, SIGH <br />SUBJECT: Inspection of 6/22/04 <br />On 6/22/04 we were cited for violation of the California Health & Safety Code <br />(HSC) regarding treatment and storage of medical waste at our facility. <br />Specifically, we failed to provide proper documentation when a processed load <br />failed and had to be rerun. <br />In order to ensure that no untreated or improperly treated loads go to the <br />landfill, the Housekeeping department will perform the following procedures: <br />1. Any time a load fails or needs to be rerun, a department supervisor will be <br />notified by calling 468-6893. <br />2. The supervisor will review and initial the steam sterilizer chart <br />corresponding with the failed or rerun load. <br />3. The supervisor will ensure that it is clearly documented why a load did not <br />pass and the results of a rerun. <br />Housekeeping Policy#7.11A will be revised to reflect the above changes.
