Went back to the office to LINHARES <br />research the history of the facility. Per Envision, the facility is inactive. The last <br />routine inspection was conducted 4-14-2017 by Melissa Nissim. I asked Michelle <br />Ozuna in Accouning when the facility was inactivated (not on the LOG2). Per <br />Michelle, it was inactivated by the owner 9-2017. I went to the facility today and the <br />current owner Srun Hkecksim stated she has been the owner since May 2022 (but <br />she is the second owner since the owner on record from 2017 Vuth Keo). She <br />stated she didn't know she needed a health permit. I informed her that we will <br />need to conduct a change of ownership consultation inspection. She stated she <br />will fill out an SR and send it to me. She made payment in my presence Conf <br />#153897591. Awaiting SR.
