A Cbr- <br />Activity Summary <br />Activity Date <br />07/07/2022 Received page at 7:30am from general hospital who stated that 25-gallons of <br />diesel released from a big rig accident reported by CHP. Called CHP to get <br />information. Per CHP dispatch it was a solo big rig accident, no waterways were <br />impacted and it has been contained, the release occurred in the dirt on the <br />shoulder and Caltrans is on the way. Was provided the Sergeant on site's name <br />and phone number. Called Srgt. Kyle Plant who stated that driver possibly fell <br />asleep and drove off into the shoulder causing the release into the dirt shoulder. <br />The driver is okay but the tank was ruptured and the truck is currently in a tree. <br />Per Srgt. Plant the responsible party if Mountain Valley Express a corporate carrier <br />but he will had all that information on site for EHD. <br />Recorded by <br />PRESTO <br />07/07/2022 Discussed complaint with lead senior. Discussed complaint with day time on call PRESTO <br />inspector. <br />07/07/2022 EHD received CalOES spill report (#22-3855) on 7/7/2022. Spill occurred on the GARCIA-MEJIA <br />right shoulder, NB 99 just south of the Jacktone exit in Ripon, CA. <br />General Logistics Systems US, Inc. (GLS) is the responsible party. Observed <br />staining on the soil in the ditch measuring approximately 10 feet by 4 feet. Per <br />Stephanie of GLS, All Environmental Services was hired to conduct clean up. Per <br />Brandon Espinoza of All Environmental Services, they will be tarping the area, <br />collecting a soil sample for testing and marking it for USA dig. <br />Notice to abate was emailed to Stephanie Montanez at <br />stephanie.montanez@gls-us.com. Received photos of tarp placed on the spill and <br />site markings for USA dig from Juan Armendarez via text. Generated Prop65 and <br />provided to Genesis to be mailed out. <br />07/14/2022 <br />07/19/2022 <br />Saved email received from Juan Armendarez of CalTRANS indicating that cleanup <br />by All Environmental will be conducted on Tuesday 7/19/22 at 9am. <br />Arrived on site to witness final clean up. Juan Armendarez of CalTRANS & <br />Brandon Espinoza with All Environmental Services were on site. Per Brandon, <br />approximately 15-20 yards of contaminated soil were excavated. Waste will be <br />disposed of as non-hazardous and will be taken to Fairmead Landfill in <br />Chowchilla. <br />GARCIA-MEJIA <br />GARCIA-MEJIA <br />Saved photos to the public drive and printed the photos received from Juan <br />Armendarez via text message.
