OBSERVATION: The designated operator inspections for the following months were performed more than 30 days after the previous inspection: - Initial inspection on 8/23/2022, next inspection performed on 10/2/2022(40 days) REGULATION GUIDANCE: The designated operator visual inspections shall be conducted within 30 days or less of the previous inspection. CORRECTIVE ACTION: Ensure that all designated operator inspections are performed within the required time frames. OBSERVATION: Designated operator reports were not signed within the required time frame. The following designated operator report was not signed by the owner/operator: -4/7/2023, 3/20/2023, 2/13/2023, 1/19/2023, 12/15/2022, 11/22/2022, 10/21/2022, 10/2/2022, 8/23/2022, 7/27/2022 REGULATORY GUIDANCE:Within 48 hours of the completion of the designated UST operator visual inspection, the designated operator shall sign and provide the owner or operator with a copy of the inspection report. Within 72 hours of being provided a signed copy of the report,the owner or operator shall provide a description of each corrective action taken or to be taken and shall sign and date the report, acknowledging the identified compliance issues. CORRECTIVE ACTION: Ensure all designated operator inspection reports are completed and signed within the required time frame in the future. This is a repeat violation, Class II. Overall Inspection Comments: Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement documenting the corrective actions that have been or will be taken for each violation, and any supporting paperwork, by 5/27/2023. ***Note: Positive shut down was turned on on 4/28/2022. No line testing was conducted during the inspection. To minimize person to person contact EHD is choosing to write the name of person receiving the report instead of having them sign.
