S A N J D AR I N Environmental Health Department <br /> Y AMENDED <br /> 3/28/2023 <br /> Hazardous Materials Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> Razi Petroleum Inc. 950 W ELEVENTH ST, TRACY August 02, 2022 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 11 HSC 25505(a)(4) Failed to provide or document initial and/or refresher training to appropriate personnel. <br /> OBSERVATION:Annual HBMP (hazardous materials business plan)training was not observed on site. <br /> REGULATION GUIDANCE: The business plan shall include provisions for ensuring that appropriate personnel <br /> receive initial and annual refresher training.All employees shall be trained in safety procedures in the event of a <br /> release or threatened release of a hazardous material, including, but not limited to,familiarity with the following <br /> plans and procedures: <br /> (A) Immediate notification contacts to the appropriate local emergency response personnel and to the unified <br /> program agency. <br /> (B)Procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br /> persons, property, or the environment. <br /> (C) Evacuation plans and procedures, including immediate notice,for the business site. <br /> This training shall be documented electronically or by hard copy and shall be made available for a minimum of three <br /> years. <br /> CORRECTIVE ACTION: Immediately provide employee training for appropriate personnel and submit a copy of the <br /> training records to the EHD. <br /> This is a Class II violation. <br /> 12 HSC 25505(c)Failed to have the HMBP readily available to facility or EHD personnel. <br /> OBSERVATION: Facility could not provide the Emergency Response Plan and the inventory information upon <br /> request. <br /> REGULATION GUIDANCE: The emergency response plans and procedures,the inventory of information required <br /> by this article, and the site map required by this section shall be readily available to personnel of the business or the <br /> unified program facility with responsibilities for emergency response or training pursuant to this section. <br /> CORRECTIVE ACTION: Immediately make arrangements to have the business plan readily available for review. <br /> This is a minor violation. <br /> Overall Inspection Comments: <br /> An inspection report was provided to the facility operator on the day of inspection. The report was amended the <br /> same day as the inspection to add item #12 and replaces the initial report. The 8/2/2022 report was amended <br /> 8/3/2022 to correct the amended date from "8/1/2022"to "8/2/2022". The report was amended today <br /> (3/28/2023)to reflect the updated name of the business. No additional changes have been made. At the time of <br /> amendment, all violations have already been closed.
