Complaint Investigation Summary <br /> Report # 5105 <br />COMPLAINT ID: C00054924 Site Location: 12150 E ADA AVE <br />Activity Summary <br />Activity Date Recorded by <br />11/14/2022 12150 E Ada Ave -Reviewed Manure Management & Fly Control Plan. Received SINGH <br />a call from Albert Mora, Maintenance Supervisor. Mr. Mora stated the farm was <br />sold to Howard Chan, owner, approximately 8 to 9 years ago. When I asked him <br />about the disposal of wastewater from the property, he mentioned he had a <br />difficult time being permitted with City of Stockton. Currently, the facility is <br />disposing in Sacramento. Mr. Mora does not know what to do with the waste <br />water. I informed him to contact the Water Board. When asked about the waste <br />from the tank, Mr. Mora stated an employee accidentally broke the tank while <br />removing it. The waste leaked into the ground. Mr. Mora also stated he will look for <br />the Manure Management and Fly Control Plan. I informed him the EHD has a copy <br />and it can be emailed to him. Mr. Mora's work schedule is Friday to Tuesday from <br />4 am to noon (off Wednesday and Thursday). <br />Discussed with Natalia S. <br />12/01/2022 12150 E Ada Ave - Received an email from Dan G., Water Board, regarding SINGH <br />scheduling an inspection at the site. I responded to his email. Discussed with <br />Natalia S. <br />12/05/2022 12150 E Ada Ave - Received correspondence from the Water Board. Responded SINGH <br />to the email. Discussed with Natalia S. <br />12/12/2022 12150 E Ada Ave - Reviewed complaint, Manure Management Plan and the SINGH <br />Poultry General Order. <br />12/12/2022 12150 E Ada Ave, Stockton - Complaint inspection. The inspection was conducted SINGH <br />with Natalia S. (Program Coordinator), Mike S. (REHS-Unit II), Charles Cardenas <br />(Water Board) and Albert Mora (Maintenance Supervisor). The manure <br />management, pit and wash out was discussed. The excavated area (from pit <br />removal) has been filled in with soil and gravel, per Mr. Mora. The contents of the <br />pit were removed and placed in plastic bins. Mr. Cardenas permitted Mr. Mora to <br />store the bins in the barns until the barns are cleaned (after ducks are removed <br />from barns) Per Mr. Cardenas, the wash out from the barns can be combined with <br />the manure and stored on the manure pad. <br />12/27/2022 12150 E Ada Ave - Reviewed file. I emailed Albert Mora <br /><Albert@grimaudfarms.com> for photographs and the name of the hauler as <br />previously discussed. I informed him that the complaint will not be abated until the <br />information is received. Created photo log for the inspection conducted on <br />12-12-21. <br />SINGH <br />01/09/2023 12150 E Ada Ave - I called Maintenance Supervisor, Albert Mora, for an update on SINGH <br />the pictures He stated he will have them emailed. He also stated all the waste has <br />been removed from the barns. <br />Received an email from Mr. Mora stating he can not find the pictures, but he did <br />provide information for the waste hauler utilized by the facility.
