SUSZYCKI <br />11/28/2022 Visited the property and met with the tenant next to space #9. Space #9 is vacant <br />but has a large amount of surfacing sewage. According to the tenant some one <br />came out to take a look the sewage, but nothing was cleaned-up and the sewage <br />is still surfacing. <br />SUSZYCKI <br />11/29/2022 Used the pictures taken during my complaint investigation to create a <br />photographic log. Created a Sewage Disposal Notice to Abate. <br />SUSZYCKI <br />Turned the complaint in to Mr. Sangalang for review. <br />11/29/2022 Input responsible party information into Envision for billing purposes. SUSZYCKI <br />12/08/2022 MAILED REGULAR SEWAGE DISPOSAL NOTICE TO ABATE, WITH PHOTO BALLWAHN <br />LOG, DATED 11/28/2022 TO PROPERTY OWNER. <br />GOLDEN OPPORTUNITY I HOLDINGS LP <br />RE: SHADY ACRES PROP LLC <br />4900 SANT AANITA AVE STE 2C <br />EL MONTE CA 91731-1490 <br />12/16/2022 Reviewed the complaint and I did not see any attempt made by the owner to reach <br />out to Environmental Health or correct the issue. <br />SUSZYCKI <br />12/21/2022 Visited the property and observed that the surfacing sewage has stopped and the <br />contaminated dirt appeared removed. <br />SUSZYCKI <br />12/27/2022 Received a call from the owner. Told her that I was going to abate the complaint <br />and the violation was corrected.
