Biohazard bag does not meet the standard set forth in HSC 117630. <br />- Ensure that the biohazard bag is a red disposable film bag that is impervious to moisture and tear resistant. HSC 117630 <br />The large biohazard waste bags meet both the ASTM D1709 and the ASTM D1922 standards. The small biohazard <br />waste bags do not. Replace the small biohazard waste bags with bags that meet both the ASTM D1709 and the ASTM <br />D1922 standards. Submit evidence of correction to the EHD by 4/9/2019. <br />24. Medical, biohazardous, sharps, trace chemo, pathology, or pharmaceutical waste shall be segregated and contained <br />pursuant to this section. The container shall be properly labeled. HSC 118275 <br />The pharmaceutical waste containers were not labeled with the date of first use. Submit evidence of correction to the <br />EHD by 4/9/2019. <br />24. Pharmaceutical waste is not segregated for storage. <br />Nonradioactive and non-RCRA pharmaceutical wastes, that are regulated as medical waste, shall be placed in a container or <br />secondary container labeled with the words "HIGH Ht-AW1k'fN_ClNERA1jNjMW on the lid and sides, so as to be visible <br />from any lateral direction. HSC 118275(a)(6)(B) <br />The pharmaceutical container in the North Station Med Room does not have the required labels. Submit evidence of <br />correction to the EHD by 4/912019. <br />Notes: <br />1) Waste Management is the licensed medical waste hauler for the facility for both biohazardous and pharmaceutical <br />waste. Biohazardous waste is picked -up once per week. Pharmaceutical waste was picked -up quarterly in 2017 and <br />only once in 2018 per the shipping records available onsite. <br />2) The Medical Waste Management Plan has the required information, and annual training records were available for <br />review onsite. <br />3) Biohazardous waste containers are located in the storage area at the nursing stations and in the locked outside <br />Biohazardous Waste Storage which has the required signage. <br />4) Sharps containers are located on five nursing carts and in three showers in the facility. <br />5) There are two med rooms with pharmaceutical waste container. <br />Reinspecion on/about: A reinspection fee of $152 per hour may be charged.
